2. the type of your special form, please write it out in full, as shown below.
3. the type of material you require, e.g. high-gloss table foil, barbecue underlay, etc.
4. the required dimensions.
5. if desired, a comment or a sketch for simplified processing or special requests.
6. please send us a photo of your table for inspection.
